Admission & Discharge Procedure

We know that hospital admission can be a stressful experience for patients and their families at Saraswathi Institute of Medical Sciences (SIMS). We have designed our admission process to be simple, efficient and patient-friendly so that no one is denied medical attention on account of delay in the admission process.

Patients can be admitted to the hospital through the Outpatient Department (OPD), the Emergency Department, or by referral from a consultant. Once the decision for admission is made, our admission team guides the patient or the attendant through the registration process. A complete patient profile is built, and smooth treatment planning is made possible by collecting basic personal details, identification documents and relevant medical records.

The admission desk assists the patient in selecting the right category of room as per the availability and individual requirement. Necessary information regarding hospital policies, patient rights, visiting hours and facilities available is also shared at the time of admission. Our staff is available to answer questions and provide support throughout the process.

The hospital’s administrative team helps in arranging the necessary paperwork and getting the approvals required for patients who are covered by health insurance or corporate health programmes. This means patients and families can focus on treatment and leave the administrative formalities to us. All costs, package details or financial requirements are explained clearly to avoid confusion later on and to be transparent.

After the admission formalities are completed, the patients are taken to their respective room or ward. The nursing team completes an initial assessment, documents vital information and introduces the patient to the care environment. The treating doctor assesses the patient’s condition, medical history and investigation reports before formulating an individualised treatment plan.

In emergency admissions, the priority is medical care. Treatment is initiated immediately as required, and admission formalities are completed as soon as the condition of the patient is stable. Detailed instructions are provided before discharge concerning medications, diet, physical activity, wound care and follow-up appointments. These are the instructions particular to the person’s medical condition and recovery needs. For best results, patients are advised to follow all recommendations and follow-up visits very carefully.

During follow-up consultations, the doctor examines the patient’s recovery, obtains investigation reports if needed, evaluates treatment effectiveness and makes any required changes to medications or care plans. Patients are encouraged to talk about any symptoms, concerns or changes to their health at these appointments.

For people recovering from surgery, chronic diseases or specialised treatments, the follow-up may involve rehabilitation, physiotherapy, lifestyle advice or further diagnostic assessments. Regular monitoring can help prevent complications and promote long-term health and well-being.

Discharge Procedure

The discharge process starts when the treating doctor decides the patient is medically fit to go home. At that point the necessary medical paperwork, such as documents, prescriptions, investigation reports, and discharge summaries, is made ready and then explained to the patient or sometimes to the family members.  

Before leaving the hospital, we ask patients to finish the billing and other administrative formalities, and only after that does the healthcare team continue with guidance. The team talks through medications, home care instructions, follow-up visits, and any cautions to keep in mind during the recovery period.
